Long before modern nutritional research, advocates including Dr Norman Walker and the Gerson Therapy incorporated fresh vegetable and fruit juices into their approaches to health. These therapeutic protocols also present considerable anecdotal evidence.

A UCLA study investigated the effects of a three-day vegetable and fruit juice diet in 20 healthy adults. Researchers found significant changes in the gut microbiome, including decreased Firmicutes and Proteobacteria and increased Bacteroidetes and Cyanobacteria. Participants also showed reduced body weight and BMI, substantially increased nitric oxide levels, reduced markers of lipid oxidation and improved reported wellbeing. Some effects remained measurable two weeks later.

The same study also found:

  • Improved reported overall well-being, which remained evident two weeks after the juice intervention.
  • Fruit and vegetable juices exhibited high antioxidant activity associated with their diverse phenolic compounds.
  • A three-day juice diet produced a significant reduction in body weight and BMI, with some of the reduction maintained two weeks later.
  • Plasma and urinary nitric oxide increased substantially. Nitric oxide plays an important role in vasodilation and blood flow.
  • Markers of lipid oxidation decreased significantly, indicating reduced oxidative damage to lipids.

Research into the health effects of a vegetable and fruit juice diet

Adding mixed fruit and vegetable juice to the diet over 14 weeks was associated with improved levels of beta-carotene, vitamin C, vitamin E, selenium and folate.

A review of 22 studies found that consumption of fruit juice was associated with improvements in folate and antioxidant status, including beta-carotene, vitamin C and vitamin E, together with reductions in homocysteine and markers of oxidative stress.

Studies Focusing on Children

Children consuming 100% fruit juice have been found in population studies to have higher intakes of vitamin C, magnesium and potassium and higher overall diet quality than non-juice consumers. Juice consumers also consumed more whole fruit, while some studies found no difference in total fibre intake.

Research into fruit juice and childhood weight has produced differing findings. An earlier systematic review of research from 1995–2013 found that the evidence did not support an association between 100% fruit juice consumption and weight or adiposity in children after controlling for energy intake. A larger 2024 systematic review and meta-analysis found a small positive association between each additional daily serving of 100% fruit juice and change in childhood BMI. Serving quantity and the overall diet therefore remain relevant considerations.

Juice can contribute many of the nutrients supplied by whole fruit. Research modelling the replacement of 100% fruit juice with whole fruit in children’s diets found no difference in calorie intake and little difference in the intake of most nutrients examined. Whole fruit and vegetables nevertheless provide substantially more intact dietary fibre and have their own nutritional advantages.

Taken together, published research indicates that 100% fruit and vegetable juices can provide concentrated dietary sources of vitamins, minerals, polyphenols, carotenoids and other bioactive plant compounds. Human research has reported effects on nutrient status, antioxidant activity, blood pressure and vascular function, inflammatory and oxidative-stress markers, cognition, exercise performance and the gut microbiome, although results vary according to the juice, quantity consumed, study population and outcome measured.

Juice and whole produce are not nutritionally identical. Juicing removes much of the insoluble fibre while retaining many micronutrients and plant compounds. For many people, its practical advantage is that a wide variety and substantial quantity of fresh vegetables and fruit can be consumed conveniently in a single glass.
